Release of Liability, Privacy Disclosure and Evaluation Purpose
I acknowledge that AR Medical Marijuana, its employees, and contractors are not addressing specific aspects of my medical care, nor are they acting as my primary care provider or mental health professional. I, along with my heirs, assignees, or anyone acting on my behalf, agree not to hold AR Medical Marijuana, its principals, agents, or employees liable for any harm or responsibility resulting from my PTSD evaluation or diagnosis.
I agree that AR Medical Marijuana, its employees, and contractors shall not be held responsible for any legal issues that may arise as a result of my PTSD diagnosis or evaluation.
I understand that AR Medical Marijuana will maintain the privacy of my personal health information and will not disclose it to third parties without my consent, except as required by law. AR Medical Marijuana may use or disclose my health information for the purpose of conducting the health evaluation, obtaining payment, and/or assessing my eligibility for a medical marijuana recommendation.
I understand that the evaluation for a possible PTSD diagnosis is intended to assess my eligibility for a medical marijuana card in the state of Arkansas. The diagnosis will be based, in part or in whole, on the medical information I have provided or will provide. I acknowledge that I have not misrepresented my medical condition to obtain this diagnosis.
I understand that AR Medical Marijuana providers will not prescribe medications, provide direct therapy, counseling, psychiatric evaluations, or any other form of treatment for mental health conditions, including PTSD.
I acknowledge that any PTSD diagnosis provided by an AR Medical Marijuana physician is for internal purposes only and is not intended for use in disability or benefit applications or any other legal claims. For legal or benefit purposes, I understand that a comprehensive psychiatric evaluation by a board-certified psychiatrist is required, which is not provided by AR Medical Marijuana.
